When Angel hands Cordy the pencil and pad to sketch her vision, one shot shows the pencil and pad together in Angel's left hand. The next shot has the pencil separate in his right hand and the pad in his left.
If you slow this scene right down you will notice that at no point is both the pencil and pad actually in his right hand. The pencil is in his right hand and he rests it on top of the pad which is in his left.
