When the doctor comes in to kill Stu, Stu knocks him out. Stu escapes to the hallway where the doctor attacks. Stu is able to get the gun and shoot the doctor. Pay attention. He shoots twice, pauses then shoots again. The exit wounds on the doctor's back appear without a pause. So, when Stu finished his second shot, the doctor had three holes in him.
