During the scene where the camp celebrates Trapper John's promotion to Cheif Surgeon, notice that there are several lines of dialog spoken by Trapper (Elliot Gould), but the images on screen don't show him speaking at all.
One of the landmark points of this movie is that there is a constant berage of conversation that you don't see people say. In this particular scene, there is a great deal of conversation that you don't see spoken. I see your point but I think the idea here is overlapping conversation not necissarily spoken at the correct time shown.
