In season 1, the episode where the gang goes to the old burnt down school and locate their "permanent records", Hyde reveals Jackie's middle name to everyone as "Buelah". But in season 4, at the Hub, Jackie walks in with a T-shirt on with her initials on it, as J.S.B.
As evidenced in that same episode, Jackie HATED her middle name. It stands to reason that she'd be perfectly willing to use a "fake" set of initials to help hide the middle name she hates. Additionally, even if this was a mistake, minor inconsistencies between episodes aren't "slip ups" as defined by this site.
