Approximately 23 minutes before the end of the movie the crew is descending the mountain in the cable car. As they approach the bottom, Smith(Richard Burton) unscrews the light bulbs in the cable car.

The problem is that he is unscrewing the light bulbs with a clockwise motion which in reality would tighten the bulbs rather than loosen them. He uses the same motion on both bulbs.
