I know this is an old mini series, now on DVD but couldn't help but enter this as a really bad blooper. In the episode, when Nick Nolte is a merchant marine on a ship he encounters a real bad guy Falconetti.when Falconetti beats up Nolte's friend, Nolte goes after him, Nolte was a former boxer, gives him a bad beating..and Falconetti loses an eye. Falconetti is being taken off the ship to an ambulance and his left eye is bandage...then in later episodes he's wearing an eye patch on the right eye.
