If you have seen this episode, you will know that Pacey's girlfriend damaged the beautiful cake and Pacey is trying to do it up again... so he has his hands in the cream and they really aren't clean! The camera frames his face when he talks to his girlfriend, and then it frames his hands again when he opens the door... but now his hands are strangely very, very clean!!!
