D2: The Mighty Ducks - New Jersey Name
During the scrimage between the old Ducks and the new players, Fulton Reed's old duck jersey reads FULTON on it, instead of REED like the jersey did in the original. About 35 seconds after the scrimage starts PAUSE while the players are in the pile-up by the bench and you can see Fulton's 44 jersey read FULTON. If you look at the original movie you can see the same jersey read REED.

Minor inconsistencies between episodes/sequels isn't a slip up. It's entirely possible that Fulton chose to use a different name on his jersey from one year to the next. By way of example, my brother plays hockey in a local league. He has 4 jerseys, all the same number, all for the same team, but each with a different "name" on the back.
