Sophie's Choice was a movie from the early '80s about a women that has to decide which of her children to turn over to the Nazi's.
The term Sophie's Choice has since been used in many movies and TV shows to mean a hard decision to make.
In that Scrubs episode.
JD had to decide whether to be heartless and leave the animal to die or to try to save it and risk being late for their reservations.
