This occurs at approximately 17:25 into the movie, when Billy Costigan (Leonardo DiCaprio) is at his mother's funeral. He looks at a card that is with flowers on the casket, the card is signed "F. Costello". The camera shows a closeup of Costigan as he is leaving, then back to a closeup of the card. The picture is the same, but both the font of the pre-printed words and the handwriting of the signature on the card are different.
