This slip up occurs during episode 13 of season 4, episode titled: Piece of My Heart.

Skip to the scene where Addison is having a conversation with Callie, during lunch. Addison is eating chips. Watch the chips, they go from full chips to half chips to no chips at all and back to full, without her ever reaching into the bag to grab any. This could have easily been prevented. :]
