In the episode where Michael hits Meredith with his car, you see Michael driving the car as if you were in the car with him (a.k.a the cameraman that they always talk to). To his left side the car door is locked. As he pulls into the parking lot he looks away from the driving area and hits Meredith. The camera, which is inside of the vehicle, quickly pans up to see her slamming into the windshield. The camera pans back quickly to Michael (his hands stayed on the steering wheel) and the door is now visibly unlocked.

There was a film splice in this scene and it is detectable by the door lock.
