This slipup occurs when the Wampa attacks Luke on his Taun-Taun. After knocking Luke of the beast, the Wampa reaches up to break the Taun-Taun's neck and for a split second, we can see the arm of the puppeteer (I believe with red sleeves) inside the Wampa's arm.
