In the opening credits, the X-ray with "SCRUBS" on it is backwards, if you look @ the big white mass in the middle, it shows the heart on the wrong side.
That's the whole point. The writers of the show have commented on that before, stating that it's backwards on purpose to symbolize how inexperienced J.D. and all of the interns are when they first get to the hospital. Also, in either the 5th or 6th season, J.D.'s girlfriend Kim steps in and corrects it saying "That's been bugging me for years." It was done on purpose. It's not a slip up.
