After Hitch and Sarah Milas leave the fish place because Hitch has an allergic reaction to something, you can see that it is his right ear that swells up. Later, when Sarah is walking Hitch to her home, you see that his left side of his face is all saggy and a little discolored because of a reaction earlier. The reaction seems to have switched ears!
