In the begining of the movie, when Wendy has her premonition, the rollercoaster accident occurs from the hydrolics leaking and the video camera that Frankie had wrapping around the track which derailed the coaster. When Wendy gets scared and the people in the last car get off Frankie leaves with them which means the ride should not have killed those people.
