In an episode (I think "Officer of the Day"), Radar is getting changed. He has his shirt off and he leans over his bed to tuck his teddy-bear in.

The next shot we see is Radar's arm and shoulder tucking the teddy-bear in. Unfortunately, this version of Radar is clearly wearing his jacket.

Right after this, we see Radar again and he is clearly half naked. So it is obviously a blooper.
