In the pilot episode "Children of the Gods", everyone is sitting around the briefing table as O'Neill is talking to Carter. As the camera pans or shifts to show MAJOR Kowalski, he's wearing Captain's bars on his camouflage uniform, but when he's in dress uniform, he has a Major's gold oak leaf on it.
