In the first ever episode of Monk - "Mr. Monk and the Candidate" - Sharona finds Monk at his wife's grave. As they are walking away from the grave, there is a shot from behind a gravestone. To the left of the screen, there is obviously someone in the shot behind the gravestone who is not supposed to be there.
