Do you remember the infamous 'Legolas Trip'? This slip-up is very close to it. There is one shot near Legolas tripping which is taken from behind Gimli. As he is running, his boot falls off!
I just saw it on the extended dvd, it really looks like a shoe or a part of it... (it happens a few seconds after he says "...but what bare rock can tell")
