Lord of The Rings: Fellowship of The Ring - Stand-In
Before the passage of Carhadras, Boromir is fencing with Merry and Pippin. When they tackle him you can see Merry's face and it's obviously a dwarf stand-in for the actor.
You must not have read the Official Movie Guide, because in that it says that all the stand-ins wear masks that look like the actors' faces.

To add to what Newmoon said, not only did they have smaller people dress up and wear masks of the actors' face, that fact that they did it was never a secret, so it isn't a slip-up.

this is not really a slip-up. every one knows that they used scale-doubles for those kind of things and you can't really expect them to totally make the film perfect
