In the commercial for Prodigy internet service, (which I use so I really shouldn't knock), where Aretha Franklin is supposed to be a nurse, she pages Dr. Goretti holding the intercom button down. However, when she sings to get his attention on the intercom, you can clearly see she is not holding, (Hell, she didn't even press down), the button.
I noticed that, but I wasn't sure if the part where she wasn't holding down was on the intercom, she might have just been singing.

Yeah, she WAS definately supposed to be singing on the intercom, because in one shot you see some people in the hospital look up at a speaker, listening to her sing.

I think perhaps the point of the commercial was for her to not know she was on the PA and perhaps somehow the button was stuck.
