Just as Fezzik starts to climb up the Cliffs of Insanity, look closely and you can see that Buttercup is not a real person but a rag doll in a red dress. Her neck is limp and she has no face.
Andre the Giant has a bad back, when you see them climbing when the camera is zoomed out it isn't him, it's a stunt double. And you can tell that all three are rag dolls, not just the Princess.

Actually not everyone is a rag doll in that shot. You can see that Inigo is a real person and he moves his head very clearly.
