The mistake occurs just after the Prison Bus rolls down the embankment. You see lots of views at different times of the side window where you can see the train approaching. You can also see a pair of feet at the bottom of the window. This guy is dead, yet depending on what views you see out of the window, sometimes his feet are twisted sideways and sometimes they are straddled wide open.
