Early on in the film, when Hans and his minions are entering the Nakatomi building, they walk through a corridor and through a large open door (it looks a little like a garage door).

Eddie (played by Dennis Hayden -- see http://www.angelfire.com/biz/dennishayden/images/diehard.JPG) is on OUR left hand side of the group.

Watch him in relation to our left hand side of the final door frame they walk through, and he's clearly on a collision course with it!

The scene cuts just before he would have busted his nose.

NB - this may have been better edited in later releases of Die Hard, but my copy is ancient.
