I haven't seen the movie for a while, but maybe the first glass is his dad's and the second is for him, two glasses, four olives.

It's the same glass. The second time, it's empty. There's another glass, but it already has somthing in it both times.

This is a slip up, but not exactly as stated before. The first glass, to his right, he puts an onion into. Then the second glass, to his left, he puts a single olive into. It cuts away and then back, and he is putting a single olive into an olive-less glass. So, two glasses, one onion, two separate olives, ending up with one onion in one glass & one olive in the other.
