After Balthasar tells Romeo about Juliet's death, Romeo pushes him away and falls to his knees, screaming for Juliet. During that Romeo's shirt is unbuttoned and open, but we he stands back up, his shirt is buttoned up.

It could be that his shirt was just closed and not buttoned. When he starts to walk, you can see his shirt open at the bottom.
