OK, in the flash back in the beginning, you see his girlfriend get out and that guy kills her and knocks out Alex and puts the bloody knife in his car to frame him. However, later in the movie, she was killed after she shot the cop in the foot when they tried to rape her.
Actually, it never actually determines that she was killed at that point. Besides, there's also no rule saying that the visions dont lapse time. We can draw on the original for an example of this. Eric's entire death took about 47 seconds of film, but it would take at least 3 minutes to kill someone that completely.
