In the beginning of the movie, when Robbin Williams is out of his car, and walks into the tunnel where the wreck is, on the left side you can see on the middle brick-work of the tunnel the shadows, quite clearly for about 2 or 3 seconds the overhead mic boom.. it's a BIG shadow! How could they have missed this?!?!
