It's not uncommon in road accidents involving cyclists that the helmet is undamaged (or possibly cracked slightly) but there are severe head injuries.

In bike helmets, there are holes in them.... it's rather easily explained that some piece of sharp metal, or perhaps a rock went through one of the pre-existing holes in the helmet and into her brain.
