Watch the movie very closely! In the scene where Hannibal is carrying Clarice after she has been shot, they cut to Mason Verger (deformed guy) and his doctor. If you look closely, Verger has a zipper on the side of his head. It appears to be coming from his hair, but if you look later in the scene he isn’t wearing any hat or anything else. Hmmm…
In other words...you can see Mason's costume?

Sorry but you are wrong. Look very very closely and you will see that the zip in question is quite clearly coming from the wheelchair's head rest and not from the side of his head. This is not a 'slip up'.
