Dracula (1931) - His Eyes Lit Up!
Throughout the movie, they use pin-hole spot lights to light up Bela Lugosi's eyes in close-ups. At times, however, the pin-hole spot completely misses one eye.

Unfortunately, the landing mirror does not miss Bela Lugosi's reflection when he walks past it. (We know it is Bela's cos vampires don't have 'em!)
