Star Wars Episode 1: The Phantom Menace - Jar-jar Masking Failure
About 19 minutes and 40 seconds into "Star Wars: The Phantom Menace", the Gungan Bongo Submarine has lost power and has sunk to the sea floor, and Jar-jar starts complaining "Weesa die in here!". After Obi-wan opens the instrument panel at about 19:48, Jar-jar says "When are yousa thinking weesa in trouble?!". Jar-jar then rests his arm on the back of his seat, but then he grabs the seat with his hand, and if you look closely, you can see that even though you should be able to see his fingers clutching the seat, his fingers are obscured BEHIND the seat. Two shots later, his fingers are correctly composited into the shot. This slip-up was most likely missed by a careless masker at ILM, whose job was to make CG objects appear in front of real objects.
