ER - Tension Hemothorax
In the episode (first or second season) when Green and Lewis were air lifted to an accident site in a rural area. it's a family called the Heilys. The father was a pediatrician and thrown from the van, the mother was driving and stuck in the car and there was an 8 year old son and a newborn in the back (the new born was worked on by Benton and Abby - the pediatric surgeon benton got an elective with and Carter slept with).
Mark asks Susan to help with a tension hemothorax on the father. Susan inserts a huge needle in the father's chest to release the pressure but never removes the plunger. It would be impossible for the air in the chest to leave from a closed syringe.
