Near the end of the movie, when Darth Vader lifts up the Emperor and is about to throw him into the pit, you will notice that even though the Emperor (or the obvious dummy) is horizontal, the Emperor's hood is still attached to his head. It should have fallen back, revealing the rest of his head.
