Spaceballs - Phillip, or Irving?
Go to the part in the movie where after Dark Helmet finally captures Princess Vespa and calls her father. When he introduces Dr. Schlotkin to King Roland, he says the doctor's name is "Dr. Phillip Schlotkin". But, when Helmet shows the picture of Princess Vespa with her old nose, look at the bottom of the picture. To the right of the name "Vespa, Princess", it says that the doctor's name is "Dr. Irving Schlotkin". I think the person or people responsible for making that photo were originally told that the doctor's name was "Irving", but then the name was changed at the last minute to "Phillip" and they were not informed of the name change.

I don't think that is a slip-up, since it was a "before" picture, it is possible that it was the doctor who did the original nose job.
