Ally McBeal - The One Where Fish and Cage Go to California
In the scene where John Cage is sitting outside having a discussion with Bernedette Peters, the camera tracks up from behind Cage and he's resting his face on one hand... but when the camera switches to a shot of cage he's got both hands on his face. Then throughout the scene his hands are all over his face and nose and mouth even though he's never apparently moving. Severe continuity errors... clearly they had to re-shoot this a lot!

I noticed this as well. There are several "Cage" continuity errors in each episode. Mr. MacNichol must need a lot of takes to get it right. Oh, well, the genius of this show makes up for it!!!!
