Raiders of the Lost Ark - The Man Who Swallows a Fly
In the part of the movie where Indy encounters the Nazi caravan in the mountains (where he points the bazooka at the Nazi leader), when it shows a closeup of the Nazi leader's face, if you watch closely, you will see a fly land on the side of his face and crawl up into his mouth.

This is a documented slip-up. You can read about it on www.snopes.com. I had never noticed it myself, but apparently the filming conditions in that movie were pretty horrible & flies were constantly buzzing around the actors' mouths. Harrison Ford even got dysentery.

The film-makers knew what they were doing. John Milton's Paradise Lost names Beelzebub as Satan’s second in command - a fallen Angel. and some dictionaries think it a synonym for Satan Beelzebub is an anglicized version of the Hebrew Baal-zevuv, a god worshipped by the Philistines. Baal means Lord and Zevuv means Flies, i.e. The Lord of the Flies. Balloc, at the moment he swallows the fly, has crossed over to the dark side (allusion intended).

The fly was animated in. It can't be considered a slip up if it's done on purpose.
